Accident/Incident Reports – OSHA Reporting

- OSHA Requirement -

Smith & Associates will:

• Review all employee/supervisor accidents and incidents reports
• Write final Investigation Reports (from information provided)
• Report serious accidents to OSHA which will also include the written initial report
• Provide insurance company with any requested information regarding to an accident
